The Refinery Church | Equipment Request

It is your responsibility to contact the Refinery office regarding pick up time and drop off time. Pick up and drop off will be during office hours only. Items excluded from loaning are at the discretion of the Refinery office. Please submit this form to the Refinery office two weeks prior to the date requested.

In consideration for the privilege of borrowing equipment belonging to Refinery Church, you understand and acknowledge that you are responsible for this equipment while it is in your custody and control, and agree to return the equipment in the same condition in which it was received.
